Academic Program Evaluation (EVAL) is the form that shows the student’s academic program and progress toward completion (which courses they have completed and which ones are still needed). Faculty will use this form to advise students. This is a view only form.
This report includes academic standing, GPA and projected graduation date and it serves as the graduation audit. Note: Students enrolled in programs beginning in the 2018-2019 catalog - the courses are listed in the sequential academic order (Pathways mapping); older catalog versions look different. The report can be downloaded, printed or emailed in PDF format.
This knowledge article demonstrates how to access the EVAL form, view the student’s academic program, and export/create a PDF.
Step 1
After logging into Colleague Production and agreeing to the Disclosure of Information, click the Form Search icon, if necessary then type EVAL and press Enter or click Search.
Step 2
Type name, ID number or SSN of student then click OK. Best practice: partial name lookup. For example, owllink, bar.
Step 3
If there are multiple records, select the desired record by double-clicking the row or clicking the row then clicking Open.

If the student has changed their Academic Program this screen will appear. Select the Academic Program that is still active, in other words, the one that does not have an End Date by double-clicking the row or clicking the row then clicking Open.
Step 5
Click Save.

Click Update.

EVAL launches the Report Browser. Choose one of the options below:
- Click the Next Page button to view the next page;
- Click Save As;
- Click Export PDF (Best Practice); and
- Click the X to close the panel.
Note: It is important to go to the last page of the report and view the Other Courses section.
Step 8
Click Export PDF. Make any changes you wish to the font type and size, orientation, and paper size then click Create PDF.
Step 9
Click Download. The PDF will open in a new tab. Return to Report Browser by clicking the appropriate tab then click Close to complete the process.
Step 10
Click the X to close the panel.
Note: If there are any mistakes, most mistakes can be corrected by using the Substitution Form.
